Implementation of a pharmaceutical care program for diabetic patients in primary care and the impacts on clinical outcomes

Objective: To evaluate the impact of a pharmaceutical care program in primary care. Methods: This was a longitudinal, prospective, and interventional approach. The study was conducted at the Family Health Unit in the Curado IV neighborhood, Jaboatão dos Guararapes, Pernambuco, between September 2022 and February 2023. Results: Fifty patients were selected from the Family Health Unit, mainly female (70%), with 65% of them aged 60 or older. More than half (57.5%) administered medications without assistance and did not have a caregiver (57.5%). The most common comorbidities were type 2 diabetes mellitus (25.5%), arterial hypertension (20.8%), and dyslipidemia (14.8%), with 45% of patients presenting four comorbidities. Conclusion: Pharmaceutical care was essential for improving treatment adherence, resulting in statistically significant improvements. Furthermore, a relevant correlation was observed between adherence and blood pressure reduction, highlighting the positive impact of pharmaceutical intervention on drug therapy and clinical aspects of arterial hypertension.
