Objective: to carry out, based on documentary analysis and literary review, a reflection on the historical context of the internalization of medicine in Brazil and its main challenges and benefits for society and students. Methods: a narrative review, with a search for articles, from 2008 to 2022. Results: fourteen (14) scientific studies were selected, which pointed to increasing privatization of educational institutions to the detriment of their nationalization and the progressive internalization of this teaching, depolarizing it from capitals and large urban centers to remote regions. Regarding the obstacles to the effective internalization of medicine, there is a lack of investment in the structure of primary care, the provisional nature of the More Doctors Program (MDP), and the judicialization of medicine. Conclusions: the process of internalizing medicine is gradual and presents many obstacles, but it has made great progress since the institution of the MDP, adding to the health of thousands of people. In this scenario, greater government incentives are still necessary to structure health services and provide greater support for professionals, promoting ways to retain these doctors in the interior regions of Brazil.
