Pharmacotherapeutic follow-up service indicators for hypertensive and/or diabetic elderly patients

Abstract

Objective: this study aims to evaluate the performance of the PF service offered to elderly patients with systemic arterial hypertension (SAH) and/or diabetes mellitus (DM). Methods: it is a descriptive, longitudinal, quantitative, and retrospective population-based study in which participants underwent Pharmacotherapeutic Follow-up based on the Dáder method between 2017 and 2019. A performance indicator tool was used for PF services provided to outpatients. The indicators analyzed were “Pharmaceutical consultations conducted,” “Medication-related problems resolved,” and “Clinical status of patients” in 54 patients with an average age of 69 years, all hypertensive, and 28 (51.85%) diabetic. The inclusion criteria for patients included age 60 years or older and a diagnosis of SAH and/or DM, being treated at a primary care pharmacy in the municipality of Diamantina, Minas Gerais. Results: when evaluating the PF service for patients with three or more consultations, a significant improvement in outcomes was identified, except for the indicator “Medication-related problems resolved.” The identified priorities for improving the quality of the service include increasing the resolution of medication-related problems and continuing pharmaceutical follow-up. Conclusion: the obtained results allowed the identification of priorities to be addressed to improve the quality of the service.
